Launch of Shropshire Multi-Agency Transition Protocol and Information Pack

Shropshire’s Multi-Agency Team have launched a Multi-Agency Transition Protocol and Information Pack to support young people with additional needs from 13 years into adulthood.

The protocol provides information and guidance for agencies and organisations throughout Shropshire such as schools, youth and health services who work together with young people who need additional support through into adulthood.

The Multi-Agency Team has also produced a comprehensive Transition Information Pack for young people and their families. The views of our young people and their families have helped shape the both the protocol and the information pack.

Councillor Aggie Caesar-Homden, Cabinet member for children and young people’s services, said:

“Transition can be a period of uncertainty for young people and their parents or carers. Sometimes they may be unsure about what to expect and what support is available for the future. Agencies and organisations throughout Shropshire have been working together to produce a multi-agency transition protocol to address these issues. The protocol places young people at the centre of transition planning and ensures that personalised transition plans are based on their needs and aspirations.”

Shropshire Council’s Corporate Director – People, David Taylor, who recently hosted the opening of the launch at Shirehall, said:

“The protocol will ensure that everyone involved in the transition process are clear about their roles and responsibilities. Professionals from education, health, social care, guidance, and further education colleges have worked together to develop an agreed set of clear actions points for agencies to follow.”
